ATTENTION PATIENTS:

As we begin reopening our office, there are some new procedures we would like to make you aware of in an effort to do all we can to keep you, our staff and our community safe.

1.   If you are ill, have a fever above 99.5, have a sore throat, have difficulty breathing or have been exposed to someone with the Flu or Coronavirus please do not enter the building and call us to reschedule.

2.  Please respect proper social distancing if you are not the person being cared for.

3.  We will ask that you use our hand sanitizer and fill out a short health questionnaire.
